Allina Health AMB Clinic: kit with mailer should be provided. Provider should include facesheet with patient billing information and the Neogenomics order form. Mail the kit directly to Neogenomics; do not send to the Central Laboratory.

Non-Allina Health sites: Send blood testing directly to Neogenomics from your facility. Contact Neogenomics at (866) 776-5907 to set up an account, if needed.
