Label Specimen:

1. Patient first and last name (Use legal name.)
2. Patient medical record number
3. Date of sample collection
4. Time of collection
5. Phlebotomist identifier

Refrigerate whole blood specimen 2-8C

Aliquot of samples are not accepted.

Ambient - immediately transport to the laboratory
