Target blood levels in heart transplant recipients (ng/mL):

0 – 3 months post transplant 12 – 15
3 - 6 months post transplant: 10 - 15
6 - 12 months post transplant: 8 - 12
>12 months post transplant: 5 - 10

Target blood levels in kidney transplant recipients (ng/mL):

0 - 3 months post transplant: 10 - 12
>3 - 6 months post transplant: 8 - 10
>6 - 12 months post transplant: 5 - 8
>12 months post transplant: 4 - 6

Patients may have an individual goal range due to risk of rejection, or adverse effects of tacrolimus
