Detecting IGH rearrangements.

IGH rearrangements are common in a wide variety of hematologic disorders, including Lymphoma and plasma cell dyscrasias.

This test can be ordered individually or as part of a Plasma Cell Dyscrasia FISH panel (see the individual panel test for more details).

  1. Collect 5 mL bone marrow in sterile Sodium heparin (Na hep) syringe.
  2. Transfer to a sterile vial/tube.

Only one specimen tube is required to perform all ordered cytogenetics testing.

The more testing requested, the closer to the optimal volume requirements is required.

  1. Collect 5 mL bone marrow in sterile unheparinized syringe.

Fluorescence in situ hybridization (FISH)

  

Normal or Abnormal with interpretation.

When ordered as part of the Plasma Cell Dyscrasia FISH panel and a rearrangement of IGH is observed, reflex FISH to determine the partner gene will be performed and includes probes for CCND1, CCND3, FGFR3, MAF, and MAFB, in order to classify the IGH abnormality into either a standard or high rick cytogenetics category according to mSMART 3.0.
